Work Environment

This Nurse Practitioner role is primarily onsite, situated in a home care setting. Key Responsibilities

• Conduct thorough health assessments and physical examinations tailored to patients of all ages, from infants to seniors.
• Order, perform, and interpret diagnostic tests, including lab work and imaging studies, to develop accurate diagnosis and treatment plans.
• Collaborate with a multidisciplinary team to create individualized patient care plans, ensuring each treatment is both appropriate and timely.
• Prescribe medications and recommend treatments based on current clinical guidelines while monitoring the patient's response to therapy.
• Provide comprehensive health education and counseling sessions for patients and their families, addressing factors such as lifestyle changes, medication regimens, and preventive health measures.
• Participate in multidisciplinary team meetings and case conferences to discuss patient progress, treatment effectiveness, and care coordination strategies.
• Document patient encounters diligently in the EHR, ensuring compliance with state regulations and maintaining high standards of patient confidentiality.
• Collaborate with other healthcare professionals, including specialists and social workers, to deliver coordinated and holistic care.

Qualifications

• A valid and unrestricted Nurse Practitioner license in the state of New York.
• A Bachelor’s degree in Nursing or a related field; Master’s degree preferred.

• Strong foundation in healthcare practices, procedures, and pertinent protocols.
