This full-time position offers a salary range of USD 115,000 to 150,000 per year, depending on experience and expertise. As a Nurse Practitioner, you will play a pivotal role in providing comprehensive and personalized primary care to adults, primarily those on Medicare, in a collaborative and supportive environment.

Work Environment

The position is located onsite at our hospital facility in Yonkers, New York, where you will work alongside a collaborative care team that includes physicians, nurse practitioners, physician assistants, medical assistants, and a dedicated medical scribe. Key Responsibilities

As a Nurse Practitioner, you will be responsible for:

• Providing exceptional primary care to patients during office visits, addressing both routine and acute health issues.
• Conducting comprehensive annual wellness visits and health risk assessments, emphasizing holistic health management.
• Coordinating care with specialists, other healthcare providers, and testing facilities to ensure seamless patient experiences.
• Leading population health initiatives, such as ensuring eligible female patients receive bi-annual breast cancer screenings.
• Assisting the care team with phone triage and outreach, responding to patient inquiries and providing necessary education on health conditions.
• Educating patients about their care plans, treatment options, and lifestyle modifications for better health outcomes.
• Participating in marketing and promotional activities to enhance community awareness of our services.
• Conducting home visits for patients needing additional support, and performing other duties as assigned.

Patient Population / Client Focus

Qualifications

To be considered for this position, candidates must possess the following qualifications:

• A Master of Science in Nursing, Doctor of Nursing Practice, or a graduate from an accredited NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license and an active DEA license.
• Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.
