Key Responsibilities

As a Family Nurse Practitioner at Brightwood, your core responsibilities will include:

• Providing high-quality primary care to patients during routine and acute healthcare visits.
• Administering annual wellness visits and health risk assessments, ensuring thorough documentation based on best practices.
• Coordinating care with various healthcare providers, specialists, and testing facilities to ensure a seamless patient experience.
• Leading population health initiatives, such as advocating for regular breast cancer screenings in eligible female patients.
• Engaging in phone triage, assisting with outreach efforts, and performing home visits as necessary.
• Educating patients on their health status, care plans, and treatment options to empower them in their healthcare journey.
• Participating actively in promotional activities to enhance community awareness of our services.
• Reporting directly to the Medical Director and collaborating closely with operational leadership to ensure quality care delivery.

Patient Population / Client Focus

Your primary focus will be on older adults on Medicare, leveraging our innovative healthcare model that emphasizes quality over quantity. Our personalized care initiatives aim to provide holistic treatment options and preventive care strategies tailored to the specific needs of our diverse patient population. You will play a vital role in helping patients navigate their healthcare options, contributing to meaningful long-term outcomes.

Qualifications

To excel in this role, candidates must meet the following qualifications:

• A Master of Science in Nursing, Doctor of Nursing Practice, or a completed Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license and an active DEA license.
• Experience in primary care settings, especially internal medicine, geriatrics, or family medicine.
• Knowledge of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs) is essential.

Preferred Experience

Compensation and Benefits

The annual salary for this role ranges from USD 115,000 to 150,000, commensurate with experience and qualifications. We offer a competitive benefits package that includes:
