Work Environment

The work setting is within a hospital facility that values collaboration and continuous improvement. You will be part of a dynamic team of healthcare professionals dedicated to delivering exceptional care to patients. Our work schedule generally spans from 9:00 a.m. to 6:00 p.m.; however, you may occasionally work evening clinics until 8:00 p.m. Participation in weekend community outreach events or health fairs may also be required, ensuring that our services reach those who need them most. You can expect to work an average of 40 hours per week.

Key Responsibilities

• Conduct thorough health assessments and screenings, ensuring that each patient receives individualized care tailored to their unique needs.
• Diagnose and manage acute and chronic medical conditions, promoting preventive care and health education to improve patient outcomes.
• Collaborate with local community organizations to develop and implement educational health programs that address pressing health issues and promote wellness.
• Provide culturally sensitive care, taking into account the background and beliefs of each patient while advocating for their rights and access to healthcare services.
• Coordinate referrals and follow-up care, ensuring seamless transitions and continuity of care for all patients.
• Maintain accurate medical records in accordance with all regulatory standards and ensure compliance with organizational policies.
• Engage in community outreach, participating in health fairs and events to raise awareness and offer critical health screenings.
• Continuously update your knowledge regarding community health trends, evidence-based practices, and relevant healthcare policies.

Qualifications

• Valid and current Nurse Practitioner certification and active state nursing license.

Compensation and Benefits
