Work Environment

This onsite position is based in Harrison, NY, within a home care setting that allows us to reach patients in the comfort of their households. You will work a standard schedule from 9:00 a.m. to 6:00 p.m., though some evenings until 8:00 p.m. and occasional weekend outreach events may be part of your responsibilities. This structured yet flexible environment enables you to form meaningful relationships with patients, allowing for tailored care that addresses their unique health challenges.

Key Responsibilities

• Conduct thorough health assessments and screenings to identify acute and chronic health concerns, ensuring a comprehensive understanding of patients’ medical histories.
• Develop individualized care plans that integrate preventive care, chronic disease management, and health promotion strategies.
• Diagnose and treat various health conditions, working collaboratively with patients to establish achievable health goals.
• Engage with community partners to initiate and participate in health education workshops and outreach programs to raise awareness about prevalent health issues.
• Ensure culturally sensitive care by understanding and respecting the diverse backgrounds of patients, addressing their specific needs and preferences.
• Oversee continuity of care by coordinating referrals and ensuring follow-up on treatment plans.
• Maintain accurate and detailed patient records, complying with HIPAA and other regulatory standards, to support effective communication within the care team.
• Monitor and report community health trends and policy changes that affect our patients, adapting services accordingly to meet emerging needs.

Patient Population / Client Focus

Your focus will be on families in the Harrison community, providing care that ranges from pediatric to geriatric populations. You will serve individuals from diverse backgrounds, often with limited access to healthcare resources. Qualifications

• Current certification as a Nurse Practitioner and an active state nursing license, allowing you to practice within New York.
• A Bachelor’s Degree in Nursing, alongside a Master’s degree in Nursing from an accredited program.
• Strong interpersonal and communication skills, enabling effective collaboration with patients, families, and interdisciplinary teams.
• A commitment to lifelong learning and keeping abreast of best practices within community health and nursing.

Preferred Experience

• At least 3 years of clinical experience in community health or public health settings is strongly preferred.
• Demonstrated experience in providing care to diverse populations, particularly those facing social and economic barriers to health.
• Previous involvement in health promotion initiatives or community outreach programs is an asset.

Compensation and Benefits

The competitive salary for this position ranges from $180,000 to $200,000 per year, based on experience and qualifications.
