Key Responsibilities

• Conduct comprehensive assessments and physical examinations of patients, tailoring care plans to meet individual needs.
• Perform onsite health risk assessments, utilizing evidence-based guidelines to identify potential health issues and create preventive care strategies.
• Deliver wellness education and resources to empower patients in making informed health decisions.
• Support individuals in managing chronic conditions through effective communication and ongoing follow-up.

Patient Population / Client Focus

The FNP at Southern Healthcare Center will primarily focus on the outpatient population, providing care to a diverse range of patients, including families, adults, and individuals managing chronic health issues. Qualifications

• Master’s degree in Nursing from an accredited program.
• Current Family Nurse Practitioner License in the state of New York, with compliance to Continuing Education requirements.
• Strong knowledge of clinical guidelines and best practices in family medicine.
• Excellent communication and interpersonal skills, with the ability to maintain a positive work atmosphere.

Preferred Experience

• Experience as a Telehealth Clinician and proficiency with telecommunication technologies.
• Prior experience in outpatient primary care settings.

Compensation and Benefits

The Family Nurse Practitioner position offers an attractive salary range between USD 120,000 and 130,000 annually. In addition to competitive compensation, we provide a robust benefits package that includes:
