Work Environment

Key Responsibilities

• Conduct thorough health assessments and screenings, identifying individual and family health needs.
• Diagnose and manage both acute and chronic medical conditions, providing preventive care and health promotion activities.
• Develop and implement personalized care plans that cater to the cultural and social backgrounds of the patients.
• Collaborate with local community organizations to create and execute health education initiatives and outreach programs.
• Ensure continuity of care by coordinating referrals and follow-up treatments for patients.
• Maintain accurate medical records in compliance with healthcare regulations and organizational standards.
• Engage in outreach events and health fairs to deliver screenings, raise health awareness, and promote the services available at Recovery Solutions Center.

Patient Population / Client Focus

You will primarily cater to individuals residing within the Assisted Living Facility and broader Yonkers community, including diverse populations with varying healthcare needs. Your compassionate approach will be essential in advocating for patients' rights and facilitating equitable access to healthcare services for those facing socioeconomic barriers.

Qualifications

• A current Nurse Practitioner certification and an active nursing license in the state of New York are required.
• A Master’s degree in nursing from an accredited institution is mandatory.
• Strong clinical skills with a minimum of three years of experience in community health or public health settings.
• Excellent communication and interpersonal skills essential for patient interaction and community engagement.

Preferred Experience

• Experience in geriatrics or working with chronic illness management.

Compensation and Benefits
