Community-Oriented Nurse Practitioner - Full-Time Role

Work Environment

This position is performed onsite at our Yonkers clinic, which operates Monday through Friday, from 9 AM to 5 PM. The work atmosphere is collaborative, with a focus on community health initiatives and health education. Our clinic is surrounded by a vibrant community, where you will engage with various organizations and stakeholders to promote wellness and education.

Key Responsibilities

• Collaborate with local community organizations and stakeholders to design and implement impactful health education programs tailored to the needs of the community.
• Provide culturally competent care that respects the diverse backgrounds of patients while advocating for their rights to access comprehensive healthcare services.
• Conduct comprehensive clinical assessments, including physical examinations and diagnostic evaluations, to determine appropriate treatment plans.
• Document patient interactions and care plans meticulously in compliance with both clinic and legal standards, ensuring accuracy and confidentiality of all medical records.
• Engage actively in community outreach initiatives, including health fairs and screenings, aimed at improving public health awareness and facilitating preventive care for community members.
• Stay current with emerging health trends, best practices, and relevant healthcare policies that affect community health outcomes.

Patient Population / Client Focus

Your primary focus will be the veteran population housed in an assisted living facility, as well as their families. This unique setting will require you to employ empathy and understanding while addressing a variety of health needs, including chronic conditions, mental health support, and preventive health services. Your interventions will directly contribute to improved health outcomes and quality of life for this demographic.

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited nursing program.
• Current certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) through the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• A valid and unrestricted Nurse Practitioner (APRN) license in New York State, along with current DEA and NPI numbers.
• Exemplary clinical assessment and diagnostic skills complemented by the ability to make timely and informed decisions in a range of clinical scenarios.

Preferred Experience

• A minimum of 2 years of experience in clinical practice, with a preference for candidates who have worked in community health or public health settings.
• Exceptional interpersonal and communication skills to effectively liaise with a diverse patient population and healthcare team.
• A strong understanding of community resources available for patients, enabling you to provide informed guidance and referrals.

Compensation and Benefits

The annual compensation for this position ranges from $150,000 to $160,000, commensurate with experience and qualifications.
