Work Environment

This role is performed onsite at our well-equipped hospital facility, surrounded by a collaborative team of healthcare professionals. The work environment is energetic and community-oriented, focused on delivering patient-centered care. Your schedule will generally be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m., and participation in community health events may be required on weekends. An average workweek encompasses approximately 40 hours, ensuring a healthy work-life balance.

Key Responsibilities

As a Nurse Practitioner, you will take on the following crucial responsibilities:

• Conduct thorough health assessments and screenings for a diverse range of patients, identifying their unique healthcare needs.
• Diagnose and manage both acute and chronic medical conditions, emphasizing preventive care and health promotion.
• Develop and implement targeted health education initiatives in collaboration with local community organizations, aimed at improving health literacy and promoting wellness.
• Provide culturally sensitive care, advocating for patients' rights and ensuring they have access to necessary healthcare services.
• Coordinate referrals and follow-up care meticulously to maintain continuity in patient care and support client journeys toward better health outcomes.
• Maintain accurate and detailed medical records in compliance with all regulatory and company standards, ensuring a high level of quality and accountability in practice.

Qualifications

To be eligible for this role, candidates must hold the following qualifications:

• Current certification as a Nurse Practitioner.
• An active state nursing license in New York.

Compensation and Benefits
