Work Environment

The position is located onsite at our state-of-the-art hospital in Eastchester, NY, where you will work closely with a multidisciplinary team of healthcare professionals. Your work schedule will typically be Monday through Friday, from 9:00 a.m. to 6:00 p.m., with flexibility required for occasional evening clinics until 8:00 p.m. You may also partake in community outreach events or health fairs on weekends, contributing to our mission of promoting health awareness.

Key Responsibilities

• Perform thorough health assessments and screenings tailored to individuals and families within the community, identifying both acute and chronic health conditions.
• Diagnose medical conditions and create management plans, emphasizing preventive care and health promotion strategies.
• Collaborate with local organizations and key stakeholders to design and deliver effective health education programs that resonate with community needs.
• Advocate for patients by providing culturally sensitive care and ensuring their rights to healthcare access are respected and upheld.
• Manage referrals and follow-up treatments to guarantee continuity of care and positive health outcomes for patients.
• Maintain up-to-date, detailed medical records and documentation, adhering strictly to health regulations and standards.
• Engage in proactive community outreach by participating in health fairs and educational events to offer free screenings and vital health information.
• Stay informed about current healthcare trends and best practices, integrating evidence-based approaches into patient care.

Patient Population / Client Focus

This role primarily focuses on serving families and individuals in the Eastchester community, with a particular emphasis on underserved populations who often face barriers to accessing quality healthcare. Through your work, you will address a variety of health issues, including chronic disease management, preventive care, maternal and child health, and health promotion.

Qualifications

• Current certification as a Nurse Practitioner.
• Valid state nursing license in New York.
• Bachelor's degree in nursing is required; a Master's degree in nursing from an accredited institution is preferred.
• A minimum of 3 years of clinical experience, ideally within community health or public health settings.
• Exceptional interpersonal and communication skills, able to engage effectively with a diverse patient population.
• Commitment to the principles of social justice and health equity, particularly in advocacy for marginalized communities.

Preferred Experience

• Experience in community-based healthcare settings with a focus on family practice.
• Background in developing health education initiatives and programs.
• Familiarity with electronic health record (EHR) systems for accurate patient documentation and management.

Compensation and Benefits

We offer a competitive salary ranging from USD 180,000 to 200,000 annually, commensurate with your experience and qualifications.
