Key Responsibilities

• Conduct initial patient screenings, gather comprehensive patient histories, and perform thorough physical examinations, ensuring a holistic understanding of each patient's health needs.
• Maintain accurate and up-to-date patient records, verifying essential information at each appointment to ensure continuity of care.
• Analyze laboratory test results and radiological data to determine follow-up care and treatment strategies, engaging patients in shared decision-making regarding their health.
• Deliver high-quality medical care in alignment with organizational productivity metrics while maintaining a patient-centered approach.
• Act as a resource for patients regarding ongoing care information and counseling, providing emotional support and guidance for those facing critical health challenges.
• Educate patients on wellness practices, disease prevention, and treatment options, emphasizing harm reduction techniques to enhance patient health literacy.

Patient Population / Client Focus

This role focuses on a wide range of patients, including individuals of all ages from diverse backgrounds. The Family Nurse Practitioner will provide care to patients with various healthcare needs, from routine wellness check-ups to managing chronic illnesses. The practitioner will work closely with families to foster trust and ensure comprehensive preventive care, effectively improving overall family health in our community.

Qualifications

• Must hold a valid Family Nurse Practitioner certification in Family Medicine.
• Current, unrestricted Family Nurse Practitioner license in New York state is required.
• A Master's degree from an accredited program designed to prepare registered nurses for primary care services, or completion of an approved supplemental educational program.
• At least 2 years of nursing experience in family medicine or a related field.
• Proven ability to conduct thorough physical assessments, identify common health issues, and manage chronic conditions effectively.
• Strong communication skills with an emphasis on building trust and rapport with patients and colleagues alike.

Preferred Experience

• Familiarity with electronic health records (EHR) systems to facilitate efficient patient documentation and care coordination.
• Experience in a multidisciplinary team-based environment is highly desirable.
• Background in patient education and health promotion methods.

Compensation and Benefits

This role offers a competitive salary range of $150,000 to $160,000 per year, commensurate with experience and qualifications.
