Work Environment

This full-time position is primarily onsite, located within a school facility in Port Chester, NY. You will operate in a supportive and collaborative environment, typically working from 9:00 a.m. to 6:00 p.m., with the possibility of extended hours for evening clinics until 8:00 p.m. Participation in community outreach events and health fairs on weekends may also be required, ensuring our services are accessible to all segments of the community.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, focusing on both acute and chronic health conditions.
• Diagnose and manage a variety of medical issues while implementing preventive care strategies and health promotion programs tailored to the community's needs.
• Collaborate with local health organizations and stakeholders to develop and execute educational health initiatives that enhance awareness and knowledge of health issues.
• Provide culturally sensitive care and actively advocate for the rights of patients and their access to necessary healthcare services.
• Coordinate referrals to specialists and follow-up care to ensure patient continuity and comprehensive service delivery.
• Maintain meticulous and compliant medical records to fulfill regulatory requirements and uphold patient confidentiality.
• Engage in community outreach by participating in various events that promote health awareness and offer free health screenings to those in need.
• Continuously update your knowledge of community health trends, evidence-based practices, and policies affecting healthcare delivery.

Qualifications

• Current certification as a Nurse Practitioner with an active state nursing license.

Compensation and Benefits

We offer a competitive salary range of $180,000 to $200,000 per year, commensurate with experience and qualifications.
