Work Environment

This onsite position embodies a unique blend of private practice and community engagement within the picturesque setting of Harrison, New York. The role involves direct patient interaction in both the office and community settings, allowing for flexible, personalized healthcare delivery that can adapt to patients' lifestyles. The work schedule is structured from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. and participation in weekend health fairs or outreach initiatives.

Key Responsibilities

• Perform detailed health assessments and screenings for patients, ensuring a comprehensive understanding of their medical history and personal health needs.
• Diagnose and manage a broad range of acute and chronic medical conditions, tailoring treatment plans that incorporate preventive care and health promotion strategies.
• Collaborate closely with community organizations and key stakeholders to design and implement effective health education programs aimed at fostering community wellness.
• Deliver culturally sensitive care, advocating for patient rights and facilitating access to necessary healthcare services for underserved populations.
• Manage the coordination of referrals and follow-up care, ensuring seamless transitions across various healthcare settings.

Qualifications

• A current and valid certification as a Family Nurse Practitioner.

Compensation and Benefits
