Work Environment

This position is onsite in the vibrant community of Eastchester, NY. As part of a concierge medicine practice, you will often conduct house calls, allowing you to establish closer patient relationships. Key Responsibilities

• Lead patient consultations where you will assess and manage health conditions, order diagnostic tests, and prescribe medications as required.
• Collaborate with clinical staff and other healthcare professionals to ensure that high-quality patient care is delivered effectively and efficiently.
• Develop and implement personalized care plans tailored to meet the unique health needs and preferences of each patient.
• Utilize advanced assessment skills to educate patients and their families about health maintenance and disease prevention.
• Perform other job-related duties as assigned, demonstrating flexibility in a dynamic healthcare environment.
• Document patient interactions and care plans accurately using electronic health record (EHR) systems to maintain compliance and facilitate smooth workflow.

Patient Population / Client Focus

You will primarily serve adults with varied health concerns, from acute illnesses to chronic conditions requiring ongoing management. As a practitioner focusing on concierge medicine, fostering relationships will be vital in addressing the complexities of each individual's health narrative.

Qualifications

• Master’s Degree in Nursing or completion of a Doctor of Nursing Practice (DNP) program is mandatory.
• National certification as an NP through either the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• A current, unrestricted New York state nursing license is required.
• Authorization to work in the United States.
• Familiarity with Microsoft Office Suite and proficiency in using electronic health records (EHR) systems.

Preferred Experience

• Demonstrated experience in a clinical role, particularly in primary care or urgent care settings, is highly advantageous.
• Experience in providing home health services or telehealth consultations would be beneficial.
• Proficient in managing a diverse patient population and responding promptly to their varying healthcare needs.

Compensation and Benefits

The salary for this position ranges from $150,000 to $160,000 annually, commensurate with experience and caseload.
