Work Environment

This position takes place primarily onsite, where you will engage directly with our patients. Key Responsibilities

• Conduct comprehensive assessments of patients to establish diagnoses and develop individualized treatment plans.
• Provide thorough education regarding health management, lifestyle modification, and preventive care strategies.
• Perform examinations, order diagnostic tests, and interpret results to facilitate appropriate interventions.
• Manage follow-up care for chronic conditions and ensure continuity of care through regular house calls and telehealth options.
• Collaborate closely with other healthcare professionals to optimize patient outcomes and ensure a multidisciplinary approach to patient care.
• Maintain accurate and complete medical records, ensuring compliance with HIPAA regulations and organizational policies.

Patient Population / Client Focus

As a Nurse Practitioner in our concierge medicine setting, you will primarily serve the Yonkers community, focusing on diverse populations who may face barriers to accessing healthcare. This includes the elderly, patients with mobility challenges, and individuals from low-income backgrounds. Your work will be crucial in addressing health disparities and offering tailored health solutions.

Qualifications

• Master's Degree in Nursing from an accredited institution.
• Current Nurse Practitioner certification and an active New York state nursing license.
• A minimum of 2 years of clinical experience as a Nurse Practitioner, particularly in community health or concierge medicine settings.
• Exceptional interpersonal and communication skills, enabling effective interactions with a wide range of patients.
• Commitment to advocating for vulnerable populations, focusing on health equity.

Preferred Experience

• Experience in concierge medicine or house call services.
• Familiarity with electronic health record (EHR) systems for accurate patient documentation and management.
• Demonstrated ability to work independently as well as part of a healthcare team.

Compensation and Benefits
