Work Environment

This position is based onsite at our outpatient facility in Rye, a thriving area known for its commitment to community well-being. Your work schedule will typically be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics that extend until 8:00 p.m. You may also engage in community outreach events during weekends, contributing to the health education of the wider population. The average workweek encompasses approximately 40 hours, centered around delivering care in a dynamic and engaging setting.

Key Responsibilities

• Perform thorough health assessments and screenings for individuals and families, considering their unique backgrounds and healthcare needs.
• Diagnose and manage both acute and chronic medical conditions, offering preventive care strategies and health promotion initiatives tailored to patients' needs.
• Collaborate with local community organizations, healthcare providers, and stakeholders to develop and implement impactful health education programs.
• Provide culturally competent care while advocating for patients' rights to ensure their access to healthcare services.
• Coordinate appropriate referrals and follow-up care to maintain continuity in patient management.
• Maintain precise medical documentation that adheres to regulatory standards and best practices.
• Participate in community outreach events and health fairs to enhance public health awareness while offering health screenings.
• Stay informed about emerging health trends, evidence-based practices, and relevant healthcare policies to enhance service delivery.

Patient Population / Client Focus

The primary focus of this role encompasses a diverse patient population that includes individuals and families within the local community. As a Nurse Practitioner, you will serve a variety of demographics, ensuring that healthcare access is equitable and tailored to the specific health needs of underserved groups. You will directly influence the well-being of your patients by addressing their comprehensive healthcare needs and supporting them in navigating the complexities of the healthcare system.

Qualifications

• A Master's or Doctoral degree in Nursing from an accredited educational institution.
• Current certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Possession of a valid, unrestricted Nurse Practitioner (APRN) license in New York State, along with a current DEA and NPI number.
• Strong clinical assessment and diagnostic skills, with an ability to work independently and collaboratively.
• Effective communication and interpersonal skills necessary for working with diverse patient populations.
• Proficiency in using electronic health records (EHR) systems to track patient health data responsibly.

Preferred Experience

• Minimum of 3-5 years of clinical experience in primary care or family medicine preferred, particularly in community health or public health settings.
• Demonstrated ability to engage effectively with patients from various backgrounds, ensuring consistent and compassionate healthcare delivery.

Compensation and Benefits

The salary for this full-time position ranges from USD 120,000 to 130,000 annually , commensurate with the candidate's experience and qualifications.
