Position Overview

The BrightPath Nurse Practitioners Clinic is seeking an experienced and compassionate Nurse Practitioner to join our dedicated team in White Plains, New York. This full-time position will allow you to provide significant nursing and medical services in an assisted living facility, focusing on health promotion, disease prevention, and the management of both acute and chronic conditions. Key Responsibilities

As a Nurse Practitioner at BrightPath, you will be responsible for:

• Conducting thorough assessments of patients to determine their health needs, including physical examinations and history-taking.
• Formulating and implementing individualized care plans based on patient assessments, adhering to clinical protocols and evidence-based practices.
• Providing health education and counseling to patients and families on disease management, lifestyle modification, and preventive care.
• Monitoring patient progress and collaborating with the healthcare team to adjust treatment plans as necessary.
• Maintaining comprehensive and accurate medical records in accordance with healthcare regulations and standards.
• Participating in quality improvement initiatives aimed at enhancing patient satisfaction and clinical outcomes.
• Ensuring adherence to all prescribing and clinical guidelines to optimize patient safety and effectiveness of treatments.

Patient Population / Client Focus

Your work will primarily focus on the elderly population residing in the assisted living facility, many of whom may present with complex healthcare needs due to chronic illnesses, cognitive impairments, and age-related health concerns. You will provide empathetic care to improve their quality of life, emphasizing dignity and respect for each individual.

Qualifications

The ideal candidate will possess the following qualifications:

• A Master's degree in Nursing from an accredited program.
• Current certification as a Nurse Practitioner.
• An active Nursing license for the state of New York.
• A minimum of 3 years of clinical experience, with a strong preference for experience in community health or public health settings.
• Exceptional interpersonal and communication skills to effectively engage with diverse patient populations.
• A deep commitment to social justice, health equity, and advocacy for vulnerable and underserved communities.

Preferred Experience

While not mandatory, candidates with additional experience in geriatrics, chronic disease management, or palliative care will stand out. Familiarity with electronic health record (EHR) systems and telehealth services is also advantageous in today's healthcare landscape.

Compensation and Benefits

BrightPath offers a competitive annual salary ranging from $150,000 to $180,000, commensurate with your experience and qualifications. In addition to a robust salary, our benefits package includes:
