Key Responsibilities

• Conduct thorough patient assessments, including physical examinations and health history reviews, to develop tailored care plans that meet individual needs.
• Utilize established protocols to diagnose, treat, and manage acute and chronic health conditions while promoting preventive care practices.
• Evaluate patient progress regularly and adapt care plans in collaboration with healthcare team members, ensuring optimal health outcomes.
• Educate patients and their families about health promotion, disease prevention, and management strategies to enhance their understanding of their health conditions.
• Document all patient interactions in our electronic health record system accurately and efficiently to maintain continuity and quality of care.

• Stay updated on best practices and engage in continuing education to maintain competency and provide evidence-based care.

Patient Population / Client Focus

The focus of your practice will be on elderly residents and individuals with diverse backgrounds seeking assistance with both physical and mental health challenges. You will gain experience working with a variety of comorbidities, including cognitive disorders, chronic pain, and mobility issues, necessitating a holistic approach to care that addresses both medical and emotional needs.

Qualifications

• A Bachelor’s degree in Nursing from an accredited program.
• Current Nurse Practitioner certification, with an active state nursing license to practice in New York.
• A solid foundation in adult and geriatric care, with demonstrated clinical competencies.
• Strong interpersonal and communication skills to enable effective interactions with team members and a diverse patient population.
• A demonstrated commitment to social justice, advocating for the rights and needs of vulnerable communities.

Preferred Experience

• 3-5 years of clinical experience, especially in community health or public health settings.

Compensation and Benefits
