The Life Line Medical Care Center is seeking a dedicated and compassionate Nurse Practitioner (NP) to join our dynamic healthcare team at our Yonkers, New York location. Key Responsibilities

• Conduct comprehensive assessments of patients by performing detailed physical examinations and thorough medical histories to inform treatment decisions.
• Order and interpret laboratories and diagnostic tests, evaluating patient conditions to determine the most effective healthcare strategies.
• Develop, implement, and manage individualized treatment plans based on patient evaluations, evidence-based practices, and collaborative discussions with physicians and other members of the healthcare team.
• Educate patients and their families on medical care processes, management of health conditions, medication regimens, nutrition, and health promotion strategies, enhancing their understanding and adherence to treatment goals.
• Maintain meticulous documentation of patient treatment plans and outcomes in compliance with regulatory standards, ensuring a high standard of care and communication.
• Provide specialist-level consultations for health issues pertinent to your area of expertise, offering insight and therapeutic options to patients in need.
• Perform minor invasive procedures as permitted under your practice scope upon delegation from the supervising physician, ensuring patient comfort and safety.

Patient Population / Client Focus

As a Nurse Practitioner, you will primarily serve elderly residents within the assisted living facility, many of whom may have complex medical needs due to chronic conditions. Your empathetic and patient-centered approach will be essential in managing both acute and chronic health issues, ensuring that each resident receives tailored and compassionate care that addresses their unique situation.

Qualifications

• A Master’s or Doctoral Degree in Nursing, with a specialization in Nurse Practitioner.
• Graduation from an accredited Nurse Practitioner program and current certification as a Nurse Practitioner.
• A valid Nurse Practitioner license in New York State, with privileges to prescribe medications.

Preferred Experience

• Previous experience in an assisted living or geriatric care setting is highly desirable.
• Familiarity with electronic health record systems (EHR) to maintain accurate and efficient patient documentation.
• Experience in managing multidisciplinary teams and collaborating on patient care strategies.

Compensation and Benefits
