Key Responsibilities

As a Nurse Practitioner at Pathway Health NP Group, your day-to-day responsibilities will include:

• Conducting comprehensive health assessments of residents, including physical exams and diagnostic tests to formulate care plans.
• Diagnosing and treating both acute and chronic health conditions while emphasizing preventive care and health education.
• Collaborating with physicians, nurses, and other healthcare professionals to deliver coordinated and integrated care.
• Monitoring patient progress and adjusting treatment plans based on individual responses to therapy.
• Providing patient and family education to promote health and wellness.
• Maintaining accurate and up-to-date medical records and documentation in compliance with regulatory standards.
• Participating in regular team meetings to discuss patient care and contribute to continuous quality improvement initiatives.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve elderly residents who require assistance with daily living activities, medical management, and social support. This patient population may present with complex health needs, including but not limited to chronic diseases, cognitive impairment, and mobility challenges. Your expertise will contribute significantly to enhancing their quality of life, addressing both medical and psychosocial aspects of care.

Qualifications

• Successful completion of a Master's degree in Nursing from an accredited institution.
• Current certification as a Nurse Practitioner in the state of New York.
• Active and unrestricted state nursing license.

Preferred Experience

• A minimum of three years of clinical experience in a healthcare setting, ideally within community health, geriatrics, or assisted living environments.
• Demonstrated competence in handling the unique healthcare needs of geriatric patients.

Compensation and Benefits

We offer a competitive salary range of $150,000 to $160,000 annually, contingent upon experience and qualifications. In addition to salary, our comprehensive benefits package includes:
