Key Responsibilities

• Provide comprehensive, high-quality care to residents, adhering to national and community standards.
• Conduct thorough assessments and formulate individualized treatment plans based on each patient's unique health status and preferences, considering their age-specific needs.
• Foster collaboration among office staff and enhance operational efficiency through the development and performance of effective office practices.
• Act as a liaison between practice management, hospital administration, and clinical staff to ensure seamless communication and service delivery.
• Maintain compliance with healthcare regulations and standards, ensuring all practices meet necessary accreditation and legal requirements.
• Engage actively in patient management and serve as a role model for aligning practice goals with broader health system objectives.

• Ensure that all professional licenses and certifications are current and up to date.

Patient Population / Client Focus

As a Nurse Practitioner in our Assisted Living Facility, you will work with a diverse population of adults, including elderly residents with chronic conditions, mental health needs, and those requiring rehabilitation. Qualifications

• Must possess New York State certification and registration as a Family, Adult, or Pediatric Nurse Practitioner.

• Ability to gain the appropriate privileges at F. F. Thompson Health.
• Familiarity with ICD10 coding is preferred.

Preferred Experience

• Previous experience working as a Nurse Practitioner in a similar healthcare setting is strongly preferred.
