Key Responsibilities

• Conduct initial patient histories and perform thorough physical examinations to assess each resident when admitted.
• Document and formulate individualized care plans, clearly outlining treatment goals while ensuring clear communication with patients and families.
• Perform daily patient rounds to monitor progress, adjust treatment plans as necessary, and engage in real-time decision-making to improve patient outcomes.
• Order relevant medications, laboratory tests, and diagnostic studies while meticulously interpreting results to influence treatment decisions.
• Collaborate with attending physicians and other care providers to deliver synchronized care and enhance overall patient well-being.
• Prepare discharge summaries, prescriptions, and referrals while effectively educating patients on post-discharge care instructions to ensure continuity of care.
• Participate in departmental performance improvement initiatives and contribute to ongoing research and educational activities to foster professional development and organizational growth.
• Adhere to protocols outlined by the Nurse Practitioner Modernization Act 2015, maintaining a collaborative relationship attestation with the New York State Department of Education.
• Ensure compliance with quality standards set forth under the Americans with Disabilities Act and other regulatory requirements.

Patient Population / Client Focus

This role primarily focuses on adult residents within our assisted living facility, many of whom may have chronic health conditions requiring consistent management and individualized care approaches. The Nurse Practitioner will develop tailored strategies to meet diverse healthcare needs while promoting autonomy and dignity among residents. Understanding patient preferences and fostering collaborative care are essential to enhancing their quality of life.

Qualifications

• Master’s Degree in a Nurse Practitioner Program along with National Board Certification in the specialty area of Internal Medicine.
• A current Nurse Practitioner license and Registered Professional Nurse license to practice in New York State are mandatory.
• Proven ability to form collaborative practice agreements, submitting relevant protocols with the NYS Department of Education.
• Current Basic Life Support (BLS) certification is required to ensure readiness in emergency situations.

Preferred Experience

• Prior experience working in assisted living or geriatric settings is highly desirable to navigate the unique healthcare dynamics of this population.
• Skills in utilizing electronic health record (EHR) systems for documentation and care coordination will be advantageous.

Compensation and Benefits

The annual salary for this position ranges from USD 150,000 to 160,000, commensurate with experience and qualifications.
