Iron Gate Health & Pain Center is actively seeking a full-time Nurse Practitioner specializing in pain management to join our dedicated medical team. Work Environment

This role is primarily onsite at our state-of-the-art assisted living facility, where you will be working in a collaborative team atmosphere. The working schedule is Monday through Friday, totaling 40 hours a week, allowing for a balanced work-life dynamic. Key Responsibilities

• Conduct thorough physical examinations and meticulously document patient medical histories to understand their health context.
• Diagnose various acute and chronic illnesses, providing appropriate treatment plans that align with best practices in pain management.
• Design and implement individualized treatment plans that address the specific needs of our patients, continuously updating them based on ongoing assessments.
• Order and interpret diagnostic tests such as blood work and imaging studies, using the results to guide treatment decisions.
• Deliver patient education on disease management and prevention strategies, empowering patients to take charge of their health.
• Collaborate with a multidisciplinary team, including physicians, nurses, and therapists, to ensure synchronized and comprehensive patient care.
• Maintain accurate, up-to-date medical records in compliance with healthcare regulations, ensuring continuity of care.

Patient Population / Client Focus

Our clinic serves a wide range of individuals facing chronic pain issues, including elderly patients residing in our assisted living facility and those with complex medical histories. This role is integral in helping patients manage their symptoms effectively, facilitating their comfort and mobility while improving their quality of life.

Qualifications

• A minimum of 2 years of experience as a Nurse Practitioner with a focus on pain management and physical examinations.
• A current Registered Nurse (RN) license in the state of New York.
• Completion of a Master's Degree in Nurse Practitioner studies.
• Strong clinical skills, excellent communication abilities, and a collaborative nature that enhances team dynamics.
• Proven ability to operate independently while managing a busy caseload in a fast-paced environment.
• Current certifications in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) are required.

Preferred Experience

Experience working in an assisted living environment or with geriatric patients is highly preferred. Familiarity with electronic health record systems and telemedicine platforms will also be advantageous.

Compensation and Benefits

The Nurse Practitioner position offers a competitive annual salary ranging from USD 150,000 to 160,000, commensurate with experience.
