Work Environment

This role is based at our onsite facility, an assisted living center located in the heart of Yonkers, New York. Key Responsibilities

• Patient Assessment and Evaluation: Conduct thorough evaluations of patients presenting with pain conditions. This includes obtaining comprehensive medical histories and performing physical examinations to tailor effective pain management strategies.
• Treatment Planning and Implementation: Collaborate with a multidisciplinary team to formulate individualized treatment plans. This involves integrating various interventions such as medication management, physical therapy recommendations, and minimally invasive procedures tailored to the specific needs of each patient.
• Procedural Assistance: Assist during interventional pain management procedures. This includes supporting physicians during injections, nerve blocks, and other procedures, ensuring patient safety and comfort throughout the process.
• Patient Education and Follow-up: Engage with patients to provide education on their conditions and outline treatment options. Conduct follow-up visits to assess treatment efficacy, make necessary adjustments, and reinforce self-care practices necessary for their ongoing health and wellness.

Patient Population / Client Focus

Our patient population predominantly consists of individuals residing in assisted living facilities, many of whom experience chronic pain conditions that require expert management. Qualifications

• Active NP license in New York state is required.
• A Bachelor's Degree in Nursing and completion of a Nurse Practitioner program are mandatory.

Preferred Experience

• Prior experience in pain management is highly preferred.
• Experience in orthopedic or surgical settings will be advantageous.
• Prior outpatient practice experience as a Nurse Practitioner is strongly preferred.
