Key Responsibilities

• Conduct comprehensive patient assessments, including detailed medical histories and physical examinations, aiming to identify effective pain management strategies tailored to individual patient needs.
• Develop and implement personalized treatment plans in collaboration with an interdisciplinary team, integrating interventions such as medication management, physical therapy, and minimally invasive pain relief procedures.
• Assist in interventional pain procedures, providing essential support during injections, nerve blocks, and similar interventions, while ensuring patient safety and comfort at all times.
• Educate patients about their diagnoses, treatment options, and self-care strategies, empowering them to take an active role in managing their pain.
• Perform follow-up assessments to evaluate the effectiveness of treatment regimens, making necessary adjustments and offering ongoing support to enhance patient well-being and satisfaction.

Patient Population / Client Focus

As a Nurse Practitioner at Signify Health Care Practice, you will primarily work with patients experiencing chronic pain conditions, including but not limited to arthritis, fibromyalgia, and post-surgical pain syndromes. Your work will directly contribute to improving the quality of life for patients, helping them regain function and achieve their personal health goals. Qualifications

• Must possess a valid Nurse Practitioner license eligible for practice in New York State.
• A Bachelor’s Degree in Nursing is required.

• Exceptional communication skills, both written and verbal, to educate and support patients and collaborate with other healthcare professionals.

Preferred Experience

• Experience in pain management is highly preferred, particularly in outpatient settings.

Compensation and Benefits

The annual salary for this full-time Nurse Practitioner role ranges between USD 120,000 and 130,000, commensurate with experience.
