Full-Time Nurse Practitioner for Assisted Living Facility

This full-time position offers a competitive salary ranging from USD 150,000 to 160,000 per year, commensurate with experience and patient caseload. Work Environment

This position is based onsite at our well-equipped assisted living facility in Rye, New York. The work environment promotes teamwork and collaboration among healthcare professionals. You will work alongside a multidisciplinary team including physicians, nurses, occupational therapists, and social workers to provide comprehensive care. Key Responsibilities

• Conduct comprehensive assessments of patients, including physical exams and health histories, to inform care plans and interventions.
• Develop, implement, and evaluate individualized nursing care plans in collaboration with the healthcare team.
• Provide clear instruction and supervision to nursing staff in executing the care regimen for residents.
• Document all patient care activities, including nursing care plans and progress notes, ensuring continuity of care and compliance with regulatory standards.
• Monitor and interpret patient responses to treatment and medications, and communicate findings effectively to relevant team members.
• Participate in the development and maintenance of quality standards for nursing practice within the facility.
• Educate residents and their families about health management, medication practices, and lifestyle modifications to optimize their health outcomes.

Patient Population / Client Focus

The Nurse Practitioner will primarily work with elderly residents in our assisted living facility. Our patient population consists of individuals with diverse healthcare needs, including chronic illnesses, mobility issues, and varying degrees of cognitive function. The ideal candidate will demonstrate a patient-centered approach, cultivating relationships built on trust and respect while providing compassionate care tailored to each individual’s unique circumstances.

Qualifications

• A Bachelor’s Degree in Nursing is required; current enrollment or completion of a Master's of Science in Nursing or Doctor of Nursing Practice program is preferred.
• National certification through the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC) is mandatory.
• An active, unrestricted license to practice as a Nurse Practitioner in the state of New York.
• Proof of U.S. work authorization.
• Ability to thrive in a flexible work environment with varying schedules.

Preferred Experience

While not mandatory, previous experience in an assisted living or long-term care setting is highly desirable. Familiarity with geriatric care principles and a strong background in patient assessment will enhance your capability to excel in this role. Experience in mentoring or training nursing staff is considered an asset.
