Work Environment

As a Nurse Practitioner at Bistro Walk-in Clinic, you will work onsite within an assisted living facility, providing a supportive and dynamic work environment. Your typical work schedule will be from 9:00 a.m. to 6:00 p.m., with the occasional need for evening clinics extending until 8:00 p.m. Additionally, participation in community outreach events or health fairs may be required on weekends, reflecting our commitment to community engagement and wellness.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families to identify health risks and manage their healthcare needs.
• Diagnose and manage a range of acute and chronic medical conditions, providing both preventive and health promotion services tailored to the unique demographics of our patient population.
• Engage in collaborative work with community stakeholders to develop and implement impactful health education programs aimed at improving public health awareness and literacy.
• Provide care that is culturally sensitive, ensuring that you respect the beliefs and practices of the diverse individuals you serve, while advocating effectively for their rights.
• Coordinate patient referrals and follow-up care, maintaining communication to ensure seamless continuity of care post-visit.
• Maintain detailed patient records in compliance with applicable laws and regulations, ensuring accuracy and privacy in all documentation processes.
• Participate actively in community outreach programs and health fairs, conducting health screenings and increasing visibility of health resources available to the community.
• Stay up-to-date with community health trends, best practices in nursing, and relevant changes in healthcare legislation to optimize patient care.

Qualifications

• Current Nurse Practitioner certification and an active state nursing license in New York.
• A Bachelor’s degree in Nursing is required, with a strong preference for candidates holding a Master’s degree from an accredited nursing program.
