Full-Time Community Health Nurse Practitioner - Home Care Setting

Position Overview

We are seeking a compassionate and dedicated Community Health Nurse Practitioner to join our team at the US Veterans Health Care Clinic in Rye, New York. In this full-time role, which operates Monday through Friday from 9 AM to 5 PM, you will leverage your expertise to provide holistic healthcare services to veterans and their families.

Key Responsibilities

• Collaborate with local organizations and healthcare authorities to develop and implement tailored health education programs, ensuring that the unique needs of the community are met through active participation.
• Provide culturally sensitive and individualized healthcare to patients, effectively advocating for their rights and navigational pathways to quality healthcare services.
• Maintain thorough and compliant medical records, ensuring adherence to all applicable regulatory standards while documenting patient interactions and care provided.
• Engage with the community by participating in outreach events and health fairs, offering health screenings, education, and support to promote health awareness.
• Continuously stay updated on community health trends, evidence-based practices, and pertinent legislative policies affecting veterans' health care.

Patient Population / Client Focus

Your primary focus will be on serving veterans and their families, many of whom face unique health challenges stemming from their service. You will support patients in managing chronic conditions, navigating mental health services, and accessing preventive care. Emphasizing a compassionate, patient-centered approach, you will ensure that each individual feels valued and understood within the healthcare system.

Qualifications

• A Master's or Doctoral degree in Nursing from an accredited program is required.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP) is essential.
• A current and unrestricted Nurse Practitioner (APRN) license in New York State.
• Possession of a valid DEA# and NPI# is required for prescribing medications.
• Strong clinical assessment and diagnostic skills with a commitment to delivering high-quality care.

Preferred Experience

• A minimum of two years of clinical experience, preferably in community health or public health settings, is highly desirable.
• Demonstrated experience working with a diverse patient population and addressing various healthcare needs in a culturally competent manner.
• Exemplary interpersonal and communication skills to effectively collaborate with team members, patients, and community stakeholders.

Compensation and Benefits

We offer a competitive salary range of USD 150,000 to 160,000 annually, commensurate with your experience and qualifications.
