Work Environment

This role is primarily onsite at our outpatient office located in Rye, New York, promoting a community-focused approach to healthcare. Key Responsibilities

• Order and interpret laboratory and diagnostic tests, adjusting treatment plans based on comprehensive evaluations to ensure optimal patient outcomes.
• Maintain accurate and compliant documentation within our electronic medical record (EMR) system, ensuring all patient interactions are recorded promptly and appropriately.
• Work closely with local and regional operations and clinical leadership to align with strategic health initiatives, particularly those aimed at enhancing value-based care.
• Deliver exceptional customer service to ensure high levels of patient satisfaction and retention, embodying our commitment to quality care.
• Practice within ethical guidelines and adhere to the Scope and Standards of Practice as well as Board Certification regulations.
• Remain informed about and compliant with all applicable state and federal laws and regulations relevant to nurse practitioners.
• Engage with primary care physicians (PCPs) and other members of the interdisciplinary care team to coordinate comprehensive patient care.
• Facilitate care coordination with other providers and specialists, including those utilizing telemedicine, to ensure continuity of care.
• Conduct all required regulatory visits, including monthly, quarterly, and annual assessments, as well as acute visits when necessary.
• Participate actively in family conferences, providing advanced care planning discussions and educational resources to relatives of patients.
• Address any pharmacy recommendations or communications from pharmacists concerning resident medications.
• Be present in assigned facilities during work hours to provide adequate face-to-face coverage and address resident needs effectively.
• Assess, diagnose, and manage both chronic and acute health conditions, utilizing evidence-based practices.
• Conduct health education initiatives to promote preventive care and manage existing health issues effectively.
• Develop individualized treatment plans, clearly communicating them to residents and their caregivers.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve patient demographics that include elderly individuals and those experiencing socioeconomic challenges. Understanding the complexities of these populations is essential to tailor holistic treatment approaches. The position involves addressing a variety of health concerns, ensuring that individuals receive comprehensive and compassionate care that respects their unique situations.

Qualifications

• Current, unrestricted state license to practice as a Nurse Practitioner.
• Active DEA license to prescribe medications safely and effectively.
• Authorization to work within the U.S.A.
• Ability to adapt to a flexible environment and varying schedules as patient needs dictate.
• A strong passion for providing healthcare, especially to senior and low-income populations.
• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited NP Certificate Program.
• National certification from the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).

Preferred Experience

Candidates with experience in geriatric care and familiarity with outpatient clinical settings will be given preference, as this role requires adeptness in managing chronic conditions typical among seniors. A background in telemedicine and interdisciplinary team collaboration is also advantageous.
