Work Environment

This role is an onsite position in Jersey City, New Jersey, where you will be working in various settings, including patients' homes and community health events. The work environment is collaborative and supportive, allowing you to work closely with a diverse team of healthcare professionals to implement effective health strategies. Your schedule will primarily be from 9:00 a.m. to 6:00 p.m., with flexibility for occasional evening clinics until 8:00 p.m. Additionally, participation in community outreach events or health fairs during weekends may be required, providing you with a chance to engage directly with the community.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, identifying their healthcare needs and providing evidence-based recommendations.
• Diagnose and manage a variety of acute and chronic health conditions, promoting preventive care and overall wellness through tailored health plans.
• Collaborate with local organizations and community stakeholders to design and implement health education programs aimed at improving community health outcomes.
• Deliver culturally competent care while advocating for patients' rights, ensuring equitable access to medical services.
• Coordinate referrals and manage follow-up care, ensuring seamless transitions for patients across different healthcare services.
• Maintain accurate and detailed medical records in adherence with regulatory standards, ensuring compliance and continuity of care.
• Engage in community outreach activities and health fairs to educate the public and provide necessary health screenings.
• Stay abreast of the latest health trends, evidence-based practices, and policy changes affecting community health.

Patient Population / Client Focus

As a Nurse Practitioner at EmpowerLife Health Care Services, you will focus on a diverse patient population within Jersey City. Our clients include individuals from various demographics, particularly those residing in underserved communities. You will be tasked with delivering healthcare services to vulnerable populations, increasing their health literacy, and bridging gaps in access to essential health resources.

Qualifications

• Must hold current certification as a Nurse Practitioner and possess an active state nursing license.
• A Master’s degree in nursing from an accredited program is required.
• Strong interpersonal and communication skills with the ability to engage effectively with diverse populations.
• A commitment to advocating for social justice and health equity in your practice.

Preferred Experience

• 3-5 years of clinical experience, specifically in community health or public health settings.
• Experience in emergency medicine or urgent care can be beneficial.

Compensation and Benefits

This role offers a competitive salary range of USD 150,000 to 180,000 annually, commensurate with experience and qualifications.
