Work Environment

This position is based on-site at our outpatient office, providing a dynamic environment where you will encounter a diverse patient population daily. The workweek is structured to facilitate a healthy work-life balance, typically totaling approximately 40 hours. Your schedule will encompass shifts from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Additionally, participation in community outreach events and health fairs on weekends may be required to foster connections with the community and address public health concerns.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, identifying health risks and necessary interventions.
• Diagnose and manage both acute and chronic medical conditions, providing preventive care and health promotion tailored to our patient population.
• Collaborate closely with community organizations and stakeholders to design and execute impactful health education programs that empower patients to take charge of their health.
• Provide culturally sensitive care, ensuring that you understand and respect the unique backgrounds of each patient.
• Advocate robustly for patients' rights and access to essential healthcare services to ensure they receive the care they need.
• Coordinate referrals and follow-up care meticulously to maintain continuity of care and optimize patient outcomes.
• Maintain comprehensive and accurate medical records, adhering to all regulatory standards and ensuring quality assurance.
• Engage actively in community outreach initiatives and health fairs, providing vital health screenings and information to bridge gaps in community health knowledge.
• Stay informed about emerging community health trends, evidence-based practices, and legislative changes relevant to our services.

Patient Population / Client Focus

This role specifically targets communities that are often underserved, with a focus on advocating for vulnerable populations facing barriers to healthcare access. Qualifications

• Possess a current certification as a Nurse Practitioner with an active state nursing license.
• Hold a Master’s degree in nursing from an accredited program.
• Demonstrated minimum of 3-5 years of clinical experience, ideally in community health or public health settings.
• Exhibit strong interpersonal and communication skills capable of connecting effectively with diverse populations.
• Show a genuine commitment to social justice and health equity initiatives within the community.

Preferred Experience

Compensation and Benefits
