Work Environment

This position is primarily onsite within our hospital facility, allowing our FNP to engage directly with patients in a supportive and well-equipped environment. Typical work hours are from 9:00 AM to 6:00 PM, with occasional evening clinics extending until 8:00 PM. Participation in community outreach events and health fairs on weekends may be required, emphasizing our commitment to community health and engagement.

Key Responsibilities

• Conduct thorough health assessments, screenings, and evaluations for individuals and families to identify health risks and develop tailored care plans.
• Diagnose and manage both acute and chronic medical conditions, offering preventive care recommendations and health promotion strategies.
• Coordinate with local organizations to implement impactful health education programs, helping to bridge gaps in healthcare knowledge and access.
• Provide culturally competent care, advocating for the rights and needs of patients to ensure equitable healthcare delivery.
• Develop and maintain accurate patient records in accordance with healthcare regulations, ensuring continuity of care through effective follow-up practices.

The Family Nurse Practitioner will work primarily with underserved populations in Newark, NJ, including individuals and families facing socioeconomic challenges. Qualifications

• Current certification as a Nurse Practitioner.
• Active and unrestricted nursing license in the state of New Jersey.
