Angina Cardiology & Family Health Clinic is seeking a dedicated and experienced Family Nurse Practitioner to join our team in Newark, New Jersey. Work Environment

Our clinic operates in a modern hospital facility dedicated to patient welfare and community health initiatives. You will be part of a collaborative and friendly team, including physicians, nurses, and administrative staff, all striving towards a common goal: enhancing the overall health of our patients. The work schedule is adaptable, primarily within the hours of 9:00 a.m. to 6:00 p.m., with flexibility to accommodate evening clinics until 8:00 p.m. and potential community outreach on weekends.

Key Responsibilities

• Perform comprehensive health assessments and screenings, gathering essential patient history and data to formulate effective care plans tailored to individual and family needs.
• Diagnose and manage a spectrum of acute and chronic medical conditions, emphasizing preventive care and health promotion strategies.
• Actively collaborate with local community organizations to design and implement impactful health education programs that address the unique challenges faced by our patient population.
• Advocate for patients by providing culturally sensitive care, ensuring their rights and access to healthcare services are prioritized.
• Coordinate referrals and follow-up care to establish continuity in treatment and ensure optimal health outcomes for all patients.
• Maintain meticulous and detailed medical records, adhering to regulatory standards and best practices in documentation.
• Participate routinely in community outreach initiatives, health fairs, and educational seminars, contributing to awareness and access to health screenings.
• Keep abreast of current community health trends, evidence-based practices, and relevant healthcare policies that influence our operations.

Patient Population / Client Focus

Your role will focus on a broad demographic encompassing both children and adults, many of whom may be underserved and face barriers to accessing healthcare. As a Family Nurse Practitioner, you will be addressing conditions common within low-income and minority communities, ensuring that care delivery is not only effective but also equitable.

Qualifications

• Current Nurse Practitioner certification and an active state nursing license in New Jersey are mandatory.
• A Master’s degree in Nursing from an accredited program is required.
• Demonstrable clinical experience of 3-5 years in community health or public health settings is essential.
• Exceptional interpersonal and communication skills are crucial for success, particularly in building trusting relationships with diverse patient populations.

Preferred Experience

• Experience working in roles that require a commitment to social justice and advocacy for vulnerable communities will be advantageous.

Compensation and Benefits

This position offers a competitive salary range of $180,000 to $200,000 per year, commensurate with your experience and qualifications.
