Work Environment

This role is primarily onsite, providing opportunities for face-to-face interactions with patients in the comfort of their homes. As part of a dedicated team of health professionals, you will engage with patients in various settings, accommodating their individual health needs and preferences. Your work schedule will typically run from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Participation in community outreach events and health fairs on weekends may also be required, contributing to a collaborative effort in elevating community health standards.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients in their homes, ensuring accurate and insightful evaluations of acute and chronic health conditions.
• Diagnose and develop comprehensive care plans that address medical needs, health promotion, and preventive care strategies tailored to each patient.
• Collaborate with local organizations and stakeholders to design and implement effective health education programs that address the specific needs of the community.
• Provide culturally sensitive care, advocating for patients' rights and facilitating access to essential healthcare services.
• Manage referrals and coordinate follow-up care to maintain continuity and quality in patient treatment.
• Keep detailed records of patient interactions, treatments, and progress in line with regulatory standards, employing electronic health record (EHR) systems.
• Engage in health promotion activities, including community outreach and health fairs, to raise awareness and provide preventative screenings and education.
• Stay informed on local health trends, evidence-based practices, and relevant policy changes, ensuring that care provided aligns with best practices in community health.

Patient Population / Client Focus

In this role, you will focus on serving diverse populations within the Paterson community, which is characterized by a rich multicultural demographic. Your patients may include families, children, and elderly individuals, many of whom face barriers to accessing necessary health services. Qualifications

• A valid and current certification as a Nurse Practitioner is required.
• An active nursing license issued by the state of New Jersey.
• A Master’s degree in Nursing from an accredited institution is essential.
• Minimum of 3-5 years of clinical experience, particularly in community health settings or public health initiatives.

Compensation and Benefits

The compensation for this role offers a competitive salary range of USD 180,000 to 200,000 annually, commensurate with experience and qualifications.
