InCore Physical Therapy is excited to welcome an experienced and compassionate Physical Therapist to our team in Garfield, New Jersey. As a full-time member of our home care services, you will play a crucial role in assisting patients recover from injuries, surgeries, and various chronic conditions. Work Environment

This position is entirely onsite, with a primary focus on providing home care. Your work hours will be from Monday to Friday, 9 AM – 5 PM, allowing you to maintain a balanced professional and personal life. You will be visiting patients' homes or assisted living facilities, giving you the opportunity to create meaningful connections with them and to see firsthand how your efforts positively impact their recovery processes.

Key Responsibilities

• Conduct thorough evaluations of patients' physical abilities, including strength, mobility, and functional performance, to establish baseline metrics and set measurable goals.
• Utilize advanced diagnostic techniques and assessments to identify specific needs and devise customized treatment plans that align with each patient's recovery objectives.
• Implement various therapeutic modalities, including manual therapy techniques, therapeutic exercises, neuromuscular re-education, and state-of-the-art equipment, to facilitate effective rehabilitation.
• Educate patients and their families about injury prevention, therapeutic strategies, and caregiving techniques to foster involvement and boost recovery efforts.
• Collaborate with a multidisciplinary team, including physicians, occupational therapists, and nursing staff, ensuring a unified approach to patient care and improved health outcomes.
• Maintain accurate documentation of patient progress and interventions in compliance with healthcare regulations and organizational standards.
• Communicate regularly with patients, caregivers, and healthcare providers to facilitate coordination of care and address any emerging concerns promptly.

Patient Population / Client Focus

Your role primarily focuses on a diverse array of patients requiring physical therapy services in the home setting, including those recovering from orthopedic surgeries, individuals managing chronic illnesses, and patients in need of rehabilitation due to injury. You will be responsible for tailoring your approach to meet the unique needs of each patient, ensuring they receive the highest quality of care while maintaining dignity and respect throughout their recovery journey.

Qualifications

• A valid license to practice as a Physical Therapist (PT) in the state of New Jersey is required.
• A Bachelor's Degree in Physical Therapy is mandatory, while a Doctor of Physical Therapy (DPT) degree or equivalent will be highly regarded.
• A minimum of two years of relevant experience in a clinical or outpatient environment is essential, demonstrating expertise in physical therapy practices.
• Exceptional communication and interpersonal skills, facilitating effective engagement with patients and healthcare teammates.
• The ability to design and implement individualized treatment plans based on comprehensive evaluations.

Preferred Experience

• Experience in home health services is preferred but not required.

Compensation and Benefits

We offer a competitive salary range of $90,000 to $120,000 per year, commensurate with experience and qualifications.
