Work Environment

The Physical Therapist will work onsite in clients' homes across Teaneck and its surrounding areas. This position offers a dynamic and flexible work setting where therapists can engage closely with clients in their own environments. Navigating various home settings allows you to conduct assessments and implement therapies in a compassionate and effective manner, truly tailoring rehabilitation to each individual's needs.

Key Responsibilities

• Develop, implement, and manage personalized physical therapy treatment plans aimed at preventing disability and improving overall patient function and mobility.
• Guide and supervise physical therapy assistants in the proper execution of treatment plans, ensuring consistency and quality of care.
• Conduct thorough evaluations of clients' physical capabilities using orthopedic assessments and functional tests to inform treatment strategies.
• Analyze home environments for rehabilitation needs, providing recommendations that facilitate client independence and safety.
• Administer manual therapies, therapeutic exercises, and education to clients, helping them engage in activities that promote functional independence and wellness.
• Evaluate, fit, and adjust prosthetic and orthotic devices, coordinating recommendations with physicians for optimal patient outcomes.
• Participate in interdisciplinary team meetings, collaborating with healthcare professionals to review, develop, and adjust care plans based on patient responses to therapy.
• Document clinical records meticulously, including patient evaluations, progress notes, and discharge summaries to maintain comprehensive patient histories.
• Instruct and empower clients and family members in the use of physical therapy equipment and exercises to enhance their engagement in their recovery process.
• Remain accessible for consultation and support to ensure optimal delivery of care by Physical Therapy Assistants.

Patient Population / Client Focus

This role focuses on a diverse range of patients receiving home health care. You will provide services to individuals recovering from surgeries, managing chronic conditions, or those needing rehabilitation to restore function. Experience working with geriatric populations is a plus, as many of our clients require specialized attention to enhance mobility and independence.

Qualifications

• Must have a Bachelor's Degree from an accredited physical therapy program.
• At least one year of experience in a physical therapy role, preferably within home health or an acute care setting.
• Active state licensure in New Jersey, maintaining good standing, as well as current CPR certification.
• Possession of a valid driver's license and automobile insurance is required to travel for client visits.
• A strong passion for patient care, clinical excellence, and a commitment to improving patient outcomes.
• Exceptional communication skills to foster relationships with clients and their families.

Preferred Experience

• Experience working directly in home care settings, delivering therapy in client homes.
• Familiarity with assistive technology and rehabilitation devices.
• Experience with OASIS documentation and Medicare regulations concerning home health care.

Compensation and Benefits
