Work Environment

This position is primarily based onsite within patients' homes, allowing for a personalized approach to patient care. Key Responsibilities

• Deliver primary care services to patients, ensuring a holistic approach to their health and well-being.
• Conduct thorough initial examinations, including physical assessments and detailed patient observations, to evaluate health status.
• Develop, implement, and modify individualized treatment plans based on clinical findings and patient needs.
• Utilize and oversee medical equipment crucial for diagnostics and patient monitoring, ensuring optimal care delivery.
• Order and interpret diagnostic tests, clearly communicating results to patients and involving them in their care decisions.
• Maintain comprehensive medical records, detailing patient histories, treatment plans, and progress notes.
• Make timely referrals to specialists and community resources as necessary, ensuring continuity and comprehensiveness of care.
• Educate patients and their families about management of health conditions, medications, and preventive health measures.

The Nurse Practitioner at Oroville Medical Care System will primarily serve a diverse patient population, including elderly individuals and those with chronic health conditions. Qualifications

• Current certification as a Nurse Practitioner is essential.

Compensation and Benefits
