Eye-Deal Vision Clinic is actively seeking a dedicated and experienced Nurse Practitioner (NP) to join our caring medical team. Work Environment

This full-time position requires you to work Monday to Friday from 9 AM to 5 PM, allowing for stability with your work-life balance. The primary setting for this role is in home care; you will engage with patients in their residences, providing them with ease and comfort while tending to their healthcare needs. This setting allows for a more personalized approach to patient care, yielding rich interactions that can significantly improve patient outcomes.

Key Responsibilities

• Conduct thorough patient assessments including comprehensive physical examinations and detailed medical histories to understand individual health needs.
• Diagnose and treat both acute and chronic health conditions, employing your clinical judgment to order diagnostics and interpret results accurately.
• In collaboration with physicians, develop and implement evidence-based treatment plans tailored specifically to each patient’s medical history and lifestyle.
• Prescribe medications and manage therapeutic interventions, ensuring compliance with your scope of practice and state regulations.
• Provide education and counseling to patients and their families on health maintenance, illness prevention, and treatment plans to promote better health choices.
• Document all patient interactions meticulously in our electronic health record (EHR) system, ensuring accurate records and continuity of care.
• Participate in interdisciplinary team meetings to contribute valuable insights and engage in clinical decision-making processes.
• Adhere strictly to clinic policies, procedures, and relevant state and federal regulations to ensure a compliant, safe, and effective care environment.
• Contribute to quality improvement initiatives and engage in ongoing educational opportunities to keep abreast of the latest clinical practices.

As a Nurse Practitioner at Eye-Deal Vision Clinic, you will primarily serve an elderly and medically complex patient population, many of whom require tailored interventions within the comforts of their homes. Qualifications

• A Master’s or Doctorate degree in Nursing from an accredited Nurse Practitioner program is required.
• Current and unrestricted Nurse Practitioner license in the State of New Jersey is essential.
• Board certification in a relevant specialty (such as Family Nurse Practitioner, Adult-Geriatric Nurse Practitioner, or Acute Care Nurse Practitioner) is required.
• Possession of a DEA license and appropriate prescriptive authority is mandatory for this position.
• Proficiency in utilizing EHR systems and maintaining clinical documentation is crucial for seamless operations within our facility.

Preferred Experience

• A minimum of 2 years of relevant experience in a hospital, acute care, or home healthcare setting is preferred to enhance your effectiveness in this role.

Compensation and Benefits
