Work Environment

The selected candidate will work onsite in an outpatient office environment in Elizabeth, New Jersey. As a member of our healthcare team, you will enjoy a collaborative atmosphere backed by supportive colleagues. Your work schedule will primarily be from 9:00 a.m. to 6:00 p.m., with potential evening clinics extending until 8:00 p.m., as well as participation in community outreach events and health fairs on weekends. Expect to dedicate approximately 40 hours per week to community health initiatives.

Key Responsibilities

• Conduct thorough health assessments and screenings, ensuring that every patient receives personalized care that addresses their medical history and current health status.
• Diagnose and manage a variety of acute and chronic medical conditions, emphasizing preventive care and health promotion strategies tailored to individual and family needs.
• Collaborate with community organizations and health stakeholders to design and implement effective health education programs aimed at addressing specific community health concerns.
• Advocate for patients' rights and ensure access to necessary healthcare services while providing culturally sensitive care that respects diverse backgrounds.
• Coordinate referrals and follow-up care, ensuring continuity of health services and comprehensive support for patients.
• Maintain accurate and detailed medical records in compliance with state and federal regulations, utilizing electronic health record systems to streamline documentation processes.
• Engage in community outreach initiatives and health fairs to provide screenings and raise awareness about health issues affecting the community.
• Stay informed of the latest community health trends, public health policies, and evidence-based practices to enhance service delivery and patient outcomes.

Qualifications

Compensation and Benefits

The compensation for this full-time position ranges between USD 150,000 and 180,000 per year, which reflects our commitment to valuing the expertise of our Nurse Practitioners.
