Work Environment

In this onsite role, you will be embedded within a supportive medical team that emphasizes a holistic approach to patient wellness. You will operate in a friendly and collaborative atmosphere dedicated to advancing community health. Your typical work schedule will be from 9:00 a.m. to 6:00 p.m., with flexibility for occasional evening clinics until 8:00 p.m. Participation in community outreach events and health fairs on weekends may also be required, amounting to an average of 40 hours per week.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, utilizing advanced diagnostic tools and evidence-based practices.
• Diagnose and manage a variety of acute and chronic conditions, ensuring to implement preventive care and health promotion strategies tailored to each patient.
• Work closely with community organizations to develop and execute health education initiatives that address pain management and general wellness.
• Provide culturally sensitive care and actively advocate for patients to ensure their rights and access to medical services are upheld.
• Coordinate referrals and ensure follow-up care is seamless, maintaining a continuum of care for all patients treated.
• Maintain accurate and detailed medical records in compliance with all regulatory standards and organizational policies.
• Engage in community outreach through health fairs, providing screenings and health information to the public to enhance community health literacy.
• Stay informed on the latest community health trends, evidence-based practices, and relevant health policies to enhance your practice.

Patient Population / Client Focus

Your primary focus will be on individuals suffering from pain-related conditions, including but not limited to musculoskeletal disorders, neuropathic pain, and post-surgical recovery. By addressing both mental and physical aspects of pain, you will help patients navigate their healthcare challenges effectively. As part of this role, you will be working with diverse populations who may have differing opinions and experiences regarding pain management.

Qualifications

• Current certification as a Nurse Practitioner is mandatory.
• Valid and active state nursing license for New Jersey.
• A Master’s degree in Nursing from an accredited program is required.
• At least three years of clinical experience, particularly in community or public health settings.

Preferred Experience

• Experience specifically in pain management or related specialties is strongly preferred.
• Proven track record of working directly with community organizations and leading health education initiatives.

Compensation and Benefits
