Work Environment

This role is primarily onsite at our hospital facility, which provides a welcoming and supportive environment. The Nurse Practitioner will typically have a fixed schedule from 9:00 a.m. to 6:00 p.m., with some evening clinics extending until 8:00 p.m., and potential weekend participation in community outreach and health fairs, collectively averaging about 40 hours per week.

Key Responsibilities

• Conduct thorough health assessments by engaging patients in discussions about their symptoms and medical history, allowing for the accurate identification of acute and chronic conditions.
• Diagnose medical conditions, develop treatment plans, and manage ongoing case care in collaboration with other healthcare providers.
• Implement preventive care strategies, health promotion initiatives, and wellness education tailored specifically for our community.
• Coordinate with local health entities and community organizations to enhance access to care and boost health literacy among residents.
• Utilize electronic health record (EHR) systems to maintain precise and compliant documentation of patient encounters and treatment plans.
