Work Environment

This position is focused on home care, providing you with the opportunity to engage with patients in a familiar environment, which is crucial for delivering compassionate care. Your work schedule typically extends from 9:00 a.m. to 6:00 p.m., with the potential for occasional evening clinics that may run until 8:00 p.m. Participation in community outreach events and health fairs on weekends may also be necessary as part of our program initiatives, allowing you to network with community members and provide vital health information.

Key Responsibilities

• Conduct thorough health assessments for diverse patients, evaluating their pain management needs and creating tailored care plans.
• Diagnose and manage a range of acute and chronic medical conditions, ensuring patients receive the preventive care and health promotion necessary for optimal recovery.
• Collaborate with community organizations and stakeholders to develop and implement educational programs directed at pain management and chronic illness prevention.
• Advocate for patients’ rights, working to ensure they have access to necessary healthcare services and fostering culturally sensitive care approaches.
• Coordinate referrals and follow-up visits, consistently monitoring patient progress to guarantee continuity of care.
• Document all patient interactions meticulously, maintaining accurate medical records in compliance with regulatory and quality assurance standards.
• Engage actively in community outreach initiatives to promote health awareness, offering screenings and educational materials to improve health literacy.
• Continuously stay informed on community health trends and evidence-based practices, integrating this knowledge into patient care strategies.

Patient Population / Client Focus

As a Pain Management Nurse Practitioner, you will primarily focus on treating patients dealing with chronic pain conditions, including musculoskeletal disorders and neuropathic pain. You will work with a diverse population across Edison, addressing complex health issues while promoting equitable treatment options. Your role will also involve understanding the social determinants of health impacting your patients, ensuring a holistic approach to pain management.

Qualifications

• A current certification as a Nurse Practitioner, with an active state nursing license in New Jersey.
• A minimum of a Master’s degree in nursing from an accredited program.
• At least three years of clinical experience as a Nurse Practitioner, with a strong preference for backgrounds in community health or public health settings.
• Exceptional interpersonal and communication skills, enabling effective collaboration with patients from various backgrounds.

Preferred Experience

• Demonstrated experience in pain management, including familiarity with pharmacological and non-pharmacological interventions.

Compensation and Benefits

This position offers a competitive salary range from USD 120,000 to 130,000 annually, commensurate with your experience and qualifications.
