Work Environment

Key Responsibilities

• Conduct thorough assessments and establish accurate diagnoses for a range of acute and chronic medical conditions affecting assisted living residents.
• Develop and implement individualized treatment plans, which may include ordering diagnostic tests, recommending therapies, and prescribing medications to optimize patient health.
• Educate patients and their families about disease prevention measures, medication management, and health promotion strategies to enhance their quality of life.
• Ensure meticulous documentation of patient encounters, treatment plans, and progress notes to maintain compliant and comprehensive medical records.
• Collaborate closely with other healthcare providers to coordinate care transitions and ensure continuity of services for residents.

Patient Population / Client Focus

In your role as a Nurse Practitioner, you will primarily serve elderly patients in an assisted living setting. Qualifications

• A current and valid certification as a Nurse Practitioner, with an active nursing license in New Jersey.

Compensation and Benefits

The competitive salary for this position ranges from $150,000 to $180,000 annually, commensurate with your experience and qualifications.
