Work Environment

You will be working onsite in our outpatient office, where a collaborative ethos thrives among healthcare professionals. The work schedule is designed for a balanced lifestyle and generally ranges from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in community outreach events and health fairs during the weekends may also be required, fostering a strong community connection and enhancing healthcare awareness.

Key Responsibilities

• Conduct thorough health assessments and screenings, ensuring a clear understanding of patients' health needs.
• Diagnose and manage a wide array of acute and chronic medical conditions, while emphasizing preventive care and health promotion strategies.
• Collaborate with local community organizations, stakeholders, and healthcare providers to develop and execute robust health education programs tailored to community needs.
• Provide culturally competent care, advocating for patients' rights and their access to necessary healthcare services.
• Coordinate referrals to specialty services and ensure detailed follow-up care to maintain continuity of treatment for all patients.
• Maintain accurate and comprehensive medical records in accordance with established regulatory standards, leveraging electronic health record systems where applicable.
• Actively engage in community outreach initiatives and health fairs, promoting wellness and providing necessary health screenings to underserved populations.

This role primarily serves diverse populations within Jersey City, including many individuals from underserved backgrounds. Qualifications

• Current Nurse Practitioner certification, along with an active New Jersey nursing license.

Compensation and Benefits

We offer a competitive salary range of $150,000 to $180,000 annually, commensurate with your experience and qualifications. In addition, our comprehensive benefits package includes:
