Key Responsibilities

• Conduct thorough assessments and diagnose both acute and chronic medical conditions.
• Develop individualized treatment plans, incorporating best practices and evidence-based guidelines.
• Order, perform, and interpret diagnostic tests to inform clinical decisions.
• Prescribe medications and implement therapeutic interventions as required.
• Provide education and counseling to patients and their families regarding health promotion, disease prevention, and chronic disease management.
• Document all patient interactions and findings diligently in our electronic medical record (EMR) system to ensure continuity of care.
• Participate in quality improvement initiatives to continuously enhance patient care practices and operational efficiency.
• Stay current with developments in the healthcare sector by attending workshops, conferences, and continuing education opportunities.

Patient Population / Client Focus

The Nurse Practitioner will serve a diverse population, including adults and geriatric patients with a variety of medical needs. Qualifications

• Must possess current Nurse Practitioner certification and an active state nursing license in New Jersey.

Compensation and Benefits

We offer a competitive salary range of $150,000 to $180,000 annually, commensurate with experience and qualifications.
