Work Environment

This is a full-time, onsite position based in our Jersey City outpatient office, which fosters a collaborative and dynamic work environment. Key Responsibilities

• Conduct comprehensive assessments and accurate diagnoses for patients presenting with both acute and chronic medical conditions.
• Develop and implement individualized treatment plans tailored to patients’ needs and health goals, including ordering and interpreting diagnostic tests.
• Prescribe and manage medications, ensuring that patients understand their treatment journey clearly through patient education initiatives.
• Collaborate effectively with physicians, nursing staff, and other healthcare professionals to coordinate holistic patient care and facilitate referrals as necessary.
• Maintain thorough and accurate medical records using electronic health record systems, ensuring compliance with regulatory standards and facilitating continuity of care.
• Participate in quality improvement initiatives and stay abreast of the latest advancements in medical practices to continuously enhance patient outcomes.
• Engage with patients and their families to educate them on preventive care strategies and empower them to lead healthier lifestyles.

Patient Population / Client Focus

Your patient demographic will include a wide range of individuals, from young children to elderly adults, requiring a nuanced approach to care. Qualifications

• Current certification as a Nurse Practitioner.
• Active New Jersey state nursing license.
• A Master’s degree in Nursing from an accredited institution.
• A strong foundation in clinical practice with a minimum of 3-5 years of experience, particularly in outpatient settings.
• Excellent communication and interpersonal skills, fostering trust and rapport with patients and team members alike.

Preferred Experience

Compensation and Benefits

This position offers a competitive salary range of USD $150,000 to $180,000 annually, commensurate with experience and qualifications.
