Work Environment

As a Community Health Nurse Practitioner, you will operate predominantly in home care settings, meeting patients in their own environments. This hands-on approach allows for a thorough understanding of the unique challenges they face, facilitating tailored healthcare interventions. Your typical work schedule will generally occur from 9:00 a.m. to 6:00 p.m., with some evening clinics extending until 8:00 p.m., and occasional participation in weekend community outreach events or health fairs.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for individuals and families, ensuring to address both acute and chronic health issues.
• Diagnose and manage various medical conditions while promoting preventive care and health education tailored to community-specific needs.
• Collaborate with local organizations to design and implement health education initiatives that empower patients and improve health literacy.
• Advocate for equitable healthcare access and cultural sensitivity in patient interactions, ensuring patients’ rights are upheld.
• Coordinate referrals and follow-up care, ensuring patients receive appropriate ongoing treatment and support.
• Maintain accurate medical records in compliance with healthcare regulations, leveraging electronic health record (EHR) systems where required.
• Actively participate in community outreach, health fairs, and educational workshops to raise awareness and provide necessary health screenings.
• Stay informed about current community health trends, best practices, and legislative policies affecting healthcare delivery.

Qualifications

• Current certification as a Nurse Practitioner.
• Active and unrestricted state nursing license in New Jersey.
• A Master's degree in Nursing from an accredited program.

Compensation and Benefits

Our Community Health Nurse Practitioner role offers a competitive salary ranging from $150,000 to $180,000 annually, commensurate with experience and qualifications.
