Key Responsibilities

• Perform comprehensive assessments of adult patients, including obtaining thorough medical histories, conducting physical examinations, and ordering necessary diagnostic tests.
• Diagnose both acute and chronic health conditions, developing treatment plans that are evidence-based and tailored to individual patient needs.
• Collaborate with physicians and healthcare teams to ensure appropriate referrals and consultations for complex cases.
• Prescribe and manage medications and therapies as needed, monitoring patient responses and adjusting treatment plans accordingly.
• Educate patients on health maintenance strategies, chronic disease self-management, and preventive health measures to promote long-term wellness.
• Carry out procedural tasks within your scope of practice, ensuring that all patient interactions are safe, effective, and respectful of patient privacy.
• Maintain detailed and accurate electronic medical records (EMRs), adhering to compliance regulations and ensuring continuity of care.

Qualifications

• Current certification as an Adult Nurse Practitioner.
