This position is located onsite in our well-equipped outpatient office, where you will interact with a multidisciplinary team, including physicians, registered nurses, and administrative staff. Key Responsibilities

• Conduct comprehensive assessments and accurately diagnose both acute and chronic medical conditions through advanced clinical techniques.
• Develop, implement, and modify evidence-based treatment plans collaboratively with physicians and other healthcare providers to meet the patient's individual needs.
• Utilize clinical tools effectively by ordering, interpreting, and acting upon results from diagnostic tests, ensuring timely interventions.
• Prescribe appropriate medications and therapies, closely monitoring patients to adapt treatment strategies as needed.
• Provide education for patients and their families on disease management strategies, medication adherence, and thorough post-discharge care plans to promote recovery.
• Engage in interdisciplinary rounds to share vital patient information and collaborate on treatment plans, enhancing care coordination.
• Document all patient interactions thoroughly in our electronic medical record (EMR) system, ensuring all data is accurate and up-to-date for optimal patient care.
• Contribute actively to quality improvement initiatives and adhere to evidence-based practice guidelines set forth by the organization.
• Stay current with advancements in medical practices and healthcare regulations through continuous education and professional development.

Patient Population / Client Focus

In this role, you will serve a diverse patient population, addressing a range of health concerns and chronic illnesses. Your ability to build strong relationships with patients will be vital in promoting their engagement in care, helping them understand their conditions, and encouraging proactive health management.

Qualifications

• A current certification as a Nurse Practitioner.
• Active nursing license in the state of New Jersey.
• Bachelor's degree in nursing from an accredited institution; a Master's degree is preferred.
• At least 2 years of clinical experience in an outpatient or hospital setting, providing care for patients with varying medical issues.
• Demonstrated exceptional communication, collaboration, and interpersonal skills.
• Ability to apply strong critical thinking and problem-solving skills in fast-paced environments.

Preferred Experience

• Experience in a multi-specialty outpatient practice is a plus.
• Familiarity with electronic medical record systems to manage patient documentation effectively.
• Previous involvement in quality improvement projects or practice guidelines development.

Compensation and Benefits

We offer a competitive salary range of $120,000 – $220,000 annually, commensurate with your experience and qualifications.
