Work Environment

This position is entirely onsite, allowing you to work directly with patients in the comfort of their own homes—a unique aspect of home care. You will have the opportunity to interact closely with individuals and families, establishing trust and rapport as you navigate their healthcare journeys. Your typical work schedule will span from 9:00 a.m. to 6:00 p.m., with occasional evening clinics running until 8:00 p.m. Participation in community outreach and health fairs during weekends may also be required—providing you with ample opportunities to enhance community engagement.

Key Responsibilities

• Conduct thorough health assessments, incorporating patient histories, physical examinations, and necessary screenings tailored to individuals and families.
• Diagnose and manage both acute and chronic medical conditions, emphasizing prevention and health promotion strategies to improve overall community health.
• Collaborate with local organizations to develop health education programs that empower individuals through knowledge, addressing specific health-related topics.
• Advocate for patients’ rights and ensure they have equitable access to necessary healthcare services, demonstrating cultural sensitivity and respect for diverse values.
• Coordinate referrals and follow-up care to ensure continuity in managing patient health and wellbeing.
• Document all patient interactions meticulously, adhering to professional standards and regulatory compliance requirements.

Qualifications

• Current certification as a Nurse Practitioner, with an active nursing license in New York State.

Compensation and Benefits
